Many Brazilians dream of doing their residency in the United States, but in practice, few manage to do it. Whether because of not feeling prepared, the big financial investment, or even not knowing how the process is for that. Therefore, today we decided to clarify some doubts about the USA’s selection process to attend the medical residency.

Why do you live in the USA?

Many express their discontent with the quality of health in Brazil. Often, the practice of medicine is hampered by public and electoral policy issues. 

Thus, many seek more developed countries to practice better quality medicine. In the USA, the organization and regulation of the practice of medicine are strictly structured, and there is a body that does the detailed inspection of residency programs.

Students have active participation in the assessment and accreditation processes and constantly check the satisfaction and quality of the teaching and structure offered by the programs. Quite different from Brazil.

How does the selection process work?

The best way to confirm this is to spend time at an educational institution in the country and see residents’ day-to-day lives. Are you sure that’s what you want? Right!

The second thing you have to keep in mind is that a foreign doctor’s diploma will weigh less than a US doctor. Therefore, you need to do well with the other requirements.

One of the most important factors is the recommendations of your mentors, so we repeat: visit residency programs in the country, demonstrate experience, and make contacts.

Each program accepts three letters of recommendation, and one must be from the director of the candidate’s graduate college.

Research and publications are also a big advantage, and for some programs, it is a mandatory requirement. The candidate also needs to write a letter describing the reasons for choosing a medical career and plans.

How is the evidence?

Foreign doctors do the same test as those who live in the country. It is called Steps and is divided into 3 phases:

  1. Step 1: Evaluates the knowledge of the basic areas applied to the clinic. It is a test with about 300 questions of multiple choices answered on a computer, which can be done in accredited places. In Brazil, it can be done in: Belo Horizonte, Brasília, Curitiba, Recife, Rio de Janeiro and São Paulo. Most of the questions focus on pathology and physiology, but all areas are evaluated. Among all the tests, it is the one that carries the most weight.
  2. Step 2: This test is divided into 2 parts: Clinical Knowledge (CK) and Clinical Skills (CS). Both are scheduled separately. The theoretical content of this test is very similar to that of residency in Brazil, but with 340 questions. There are 12 consultations of the first consultation type in the practical part, and the candidate is evaluated on his knowledge, professionalism, and ethics. This test is mandatorily held in the USA.
  3. Step 3: Includes cases that reflect routine situations in the daily life of a general practitioner. It provides a final assessment to physicians who enter care without supervision by preceptors. It is unnecessary to pass this test to have the ECFMG certificate (equivalent to the validation of the medical diploma), but it is necessary for the residence permit. Thus, applicants with a J1 visa (the majority) can take the exam during their residency. Those with an H1 visa, on the other hand, must have taken this test before entering the residence. There are several books and question banks to prepare for the exams. Foreigners tend to score very well to become more competitive candidates.

Okay, now what?

After going through all these phases, the candidate will participate in the Match. In this, the participant selects the programs he is most interested in taking. Programs that are interested in him will invite you to an interview. The candidate can apply for more than one specialty at the same time.

Once the interview period is over, candidates and programs will create their preference ranking on the match website, cross-checked. On a stipulated date known as Match Day, the result of that crossing comes out, indicating where each candidate was selected. Those candidates who have not been selected have a chance to apply for programs that have not had all vacancies filled.
